1. - Use one registration form per camper (or per family for family camps). You may make copies of the form.

2. - Find the code(s) that correspond(s) to the camp(s) for which you would like to register. Write it (them) in the appropriate place on the registration form.

3. - Complete the rest of the registration form in full.

4. - Mail the form with your $75/camp session non-refundable deposit to LVR Registrar, P.O. Box 9042, Woodland Park, CO 80866-9042.

5. - Your registration will be confirmed after April 1, 2009. At that time you will receive a Camper Packet containing everything you need to know to get ready for camp. A particularly important item in your Camper Packet will be the Health History Form, which will require documentation of a physical examination within 24 months prior to the camp session (this is part of Colorado law). If the camper has not had a recent school or sports physical, you may want to schedule one now.
